System Shutdown for National Release
CMS has just announced to the ESRD Networks the decommission timeline for our current database. Dialysis facilities only should stop sending any and all patient data to the Network. Transplant facilities should continue to send patient data to the Network.
Special note to Veterans Administration dialysis units – usage of CROWNWeb is voluntary, but encouraged. If you chose not to use CROWNWeb, please continue to send your patient data to the Network as usual.
- All PARs (Patient Activity Reports) that were due 5/10 will be kept, but not entered just in case the release in June doesn’t go according to plan.
- We believe we will have all 2728 and 2746 forms that were received by 5/8 entered. At the most we expect to have only a day or two of forms not entered.
- The network will stop all data entry activity after Friday 5/11.
- The week of May 14-18 will be dedicated to final cleanup activities in preparation for the shutdown on Friday May 18th.
- Friday May 18th a final snapshot of the national patient registry will be taken for loading into CROWNWeb.
- CROWNWeb will go live in mid-June. CMS will announce the exact date next week.
QIMS Enrollment Steps Changed - Enrollment deadline May 15th
Starting today Security Officials and End User Managers can now apply at the same time as long as you follow the revised Step 1 instructions. If it has been over 30 days since you sent your application to the help desk, please contact them immediately at 1-866-288-8912 opt 4 and ask them if they have it, and if not what steps they want you to take.
QIMS Enrollment TIps
Upon receiving your emailed user ID and password, you will have 72 hours to enter QIMS, change your password and answer six security questions. If you exceed 72 hours QIMS automatically disables your account, and you must call the help desk at 866-288-8912 option 4 and tell them you missed the 72 hour deadline and they will reset your account.
After enrolling online the help desk must receive your paperwork within 30 days or your account will be automatically disabled and you will be forced to start the application process all over. Please keep in mind that the help desk is swamped with applications from all over the country and are running behind. It is possible that they may not get to your QIMS form, even if you sent the paperwork in right away. They have added extra staff last week, and we hope this doesn’t happen to you. Unfortunately if it does, you will have to start the application process all over again. If this happens to you, there will be email notification. Detailed enrollment information is listed below.

During the 2012 Heartland Kidney Pre-Conference Workshop, Heartland Kidney Network collaborated with the Kidney Community Emergency Response Coalition (KCER) to create two valuable panel presentations so attendees could gain knowledge straight from the experts regarding disaster and emergency planning. Due to the popularity of these sessions, Heartland Kidney Network is making them available to the entire renal community.
