Information for Phase 3 users
Start Enrolling for QIMS - All dialysis units must complete by 3/16/2012
Phase 3 ends February 29th and the National Rollout will start between 3/16 and 3/31. Exact date will be announced later.
Phase 3 Facilities with QIMS Active SO and EUM accounts
These Steps must be followed one at a time. You cannot continue to Step Two until you have an active SO (Security Official) or Step 3 until you have an active EUM (End User Manager) account.
- Step One Create an SO (Security Official and End User Manager) account.
- Step Two Create an EUM (End User Manager) account.
- Step Three Create your remaining End User accounts and QIMS account approval instructions for SO's and EUM's
- Training Videos for completing the SO, EUM and Regular End User accounts.
Phase 3 users should stop entering data on February 15th. All other dialysis facilities should stop sending paper forms and PARs on that same date to allow us two weeks for final entry prior to the start of the National Release. Anything received after that date will be returned to the sending facility. Transplant units should continue business as usual.

- The QIMS Security Official Site - this is where the QIMS Security Official in your facility can log in to authorize new accounts and manage or disable other user accounts for you facility(s). Only SO's can access this. EUMs use the QIMS Regular Site.
